Serious Case Reviews (SCR) and Serious Adult Review (SAR) Briefings – April & May 2018

The Manchester Safeguarding Boards (MSB) are organising a series of Serious Case Review (SCR) and Safeguarding Adult Review (SAR) learning events. These briefings are aimed at safeguarding leads or those who will cascade the learning within their organisations using the learning packs provided.

Anyone interested in applying for one of these events will need to be registered and apply via the MSB Training website training.manchesterscb.org.uk. Please note that all applications will need to be approved by your line manager.

Child E1 SCR
Date: Tuesday 24th April         Time: 9:15 (registration) 9.30 – 10.30am

Venue: Broughton Park Rugby Club 2 Houghend Crescent M21 7TA

Baby E1 died soon after birth. Baby E1’s Mother (who was 14 years old) reported that she did not know she was pregnant until labour started. Mother gave birth, but did not tell anyone until two days later and Baby E1 was found to be dead. No agencies were involved with the mother or father of E1. This SCR will explore the findings and highlight the importance of professional awareness of the Sudden Unexpected Death in Infancy guidance and detail within it.

Child F1 SCR
Date: Tuesday 24th April         Time: 10:45am (registration) 11am – 12noon

Venue: Broughton Park Rugby Club 2 Houghend Crescent M21 7TA

Child F1 died at the aged 13 years old from a heart condition that was exacerbated by morbid obesity. F1 had been obese for many years prior to the period under review. The review will include professional responses to obesity and lack of parental engagement including neglect.

Child L1 SCR
Date: Friday 18th May        Time: 9:15 (registration) 9.30-10.30am

Venue: Broughton Park Rugby Club 2 Houghend Crescent M21 7TA

Child L1 was the subject of potentially life threatening injuries when less than two months old whilst in the care of their parents. Whilst pregnant with Child L1, the mother had made disclosures to her GP practice of physical abuse by her husband of both herself and Child L1’s elder sibling who was ten months old at the time. The review will discuss professional responses to disclosures and retractions of Domestic Violence and Abuse and other features.

Adult BA SAR
Date: Friday 18th May      Time: 10:45 (registration) 11.00am – 12:noon

Venue: Broughton Park Rugby Club 2 Houghend Crescent M21 7TA

Adult BA died in December 2015 (aged 56 years old). In the final months of life Adult BA had moved from a ground floor flat tenancy which had been occupied for several years into the flat above in order to live with, and allegedly be cared for, by the perpetrator Adult B2. Adult B2 neglected and subjected BA to violence which contributed to the death of BA.

Please cascade amongst your agencies and encourage safeguarding leads and or those who will cascade learning using the learning packs to apply to attend the briefings.
