Learning Lessons from Safeguarding Adult Reviews
The Manchester Safeguarding Adults Board (MSAB) believes that findings from Safeguarding Adults Reviews (SAR) should be shared as widely as possible in order to enhance learning and inform practice.

On the 15th May 2018 the MSAB published a SAR in relation to Adult AB
“This is a very sad case in which the person who was meant to be caring for Adult AB actually subjected them to violence which contributed to their death.
Whilst several agencies came into contact with Adult AB their needs were not holistically explored and no formal assessment was completed.
Adult AB’s autonomy and self-determination were respected and agencies focussed on making referrals rather than addressing potential issues of self-neglect and AB’s vulnerability to financial abuse was not recognised.
This case has prompted greater awareness with regard to self-neglect and the vulnerability of individuals living with others who display predatory and controlling behaviour.
The MSAB accepted all the recommendations within the report and partners are ensuring that lessons learned are being shared widely.”
